I second the Tallygear! We get Bean's medical ids through Lauren's Hope. They are on the spendy side, but usually run specials and sales, and they are great quality that have lasted through a lot! We've also gotten a 'band' id through ICOE bracelets. Only one choice for medical ids, but at $5 you can't beat it!
We get our medical ID's from Petite Baubles. Very cute, very durable, not as expensive as Lauren's Hope. And for pouches.... No one but Too Sweet Boutique!!

We like Tallygear for pouches and RoadID for medical ID. The ID is "laser" engraved, so it tolerates Joe's rough and tumble ways.
